The form replaces three previous forms: the International Boarding and Disembarkation Card, the Customs Declaration Form, and the Traveler’s Health Affidavit.

- First name and middle name (if applicable).
- First surname and second surname.
- Date of birth.
- Gender (female or male).
- Country of birth.

- Unaccompanied luggage (yes or no).
- Number of minors accompanying the traveler.
- Importation of Freely Convertible Currency (FCC) exceeding 5,000 USD: amount of money.
- Value of imported miscellaneous items (footwear, clothing, food, toiletries, etc.) up to 1,000 USD.
- Items brought in luggage (details and value).
- MINCOM import permit for certain items, if applicable.
- Declaration of items to customs (yes or no).
